My baby was born at Ft. Irwin. How do I get a copy of the birth certificate?

  • Complete the Request for Certified Copy, enclose the appropriate fee, and send it to the address on the form. The certificate will be mailed to you by San Bernardino County.
  • If there is any chance that your child will ever apply for a passport, you must request the long form.

How do I get a Social Security Card for my baby that was born at Ft. Irwin?

  • You must submit your Request for Certified Copy. Upon it's receipt the Social Security Administration will issue your child a Social Security Card.

Can Weed Army Community Hospital (WACH) fax my records to another doctor or hospital?

  • No! However, you may obtain a copy of the information you need for your appointment.
  • Most civilian care providers do not require a patient's entire medical record for a visit.

Do you need a Copy of Your Medical Record?

  • Per AR 40-66 when you ETS or Retire the record remains property of the government.
  • You should request a copy of your record a minimum of 10 days prior to ETSing or Retiring.
  • You may also request copies of your family member's records a minimum of 10 days prior to when you leave the area.

Requesting Your Medical Record from A Civilian Facility or Physician

  1. First, you must obtain the complete mailing address (to include zip code)
  2. Bring the information to the Medical Correspondence Clerk who located in Bldg. S-174. The request will be processed.
  3. When the paperwork is obtained by our facility it will be placed in your medical records.

How do I request my records, paperwork, or XRays from another military facility?

  • Visit the Medical Correspondence Clerk who located in Bldg. S-174.
  • You will complete a DD Form 877
  • When the paperwork is obtained by our facility it will be placed in your medical records.

I am a lawyer, and I want a copy of a client's medical record.  What do I do?

  • Complete a DA Form 4254 (PureEdge, PDF), Request for Private Medical Records, and submit it to the Medical Correspondence Clerk.
